Abstract: Sialolithiasis is the most common disease of salivary glands and therefore causing acute and chronic infections. Major salivary gland is most commonly affected. Sialolithiasis is the main cause of unilateral diffuse swelling of parotid or submandibular gland. Submandiblar gland or its duct affects most commonly. This article discusses review of literature, pathophysiology, signs and symptoms, diagnostic methods and various modalities available for the management of sialolithiasis.

Oral submucous fibrosis (OSMF) is a chronic, insidious oral mucosal condition that occurs predominantly among Indians and occasionally in other Asians especially Taiwanese and sporadically in Europeans. It was first described by Pindborg and Sirsat.8 It is considered as a potentially malignant disorder. Although the pathogenesis of the disease is thought to be multifactorial, chewing of betel quid/areca nut has been recognized as one of the most significant risk factors for OSMF.1

30 extracted human maxillary central and lateral incisors were instrumented with K-files and filled using lateral compaction of gutta-percha (GP) and SEAL APEX sealer. The teeth were randomly divided into three experimental groups of 10 specimens each. Removal of gutta-percha was performed with the following devices and techniques: (NEW RETREATMENT FILE: EDGE FILE XR), (R-Endo retreatment files),( M TWO retreatment files) Time to reach working length and to eliminate filling material was also recorded. The specimens were rendered transparent for evaluation of the area of the remaining gutta-percha/sealer under stereomicroscope at 6X magnification. it was concluded that although MTWO RETREATMENT FILES removed root canal filling materials faster than EDGE FILE XR AND R-ENDO,but EDGE FILE XR removed maximum amount of root canal filling materials from the canal.

Patients with limited mouth opening are frequently encountered in prosthodontic practice. Microstomia defined as an abnormally small oral orifice, can be associated with various etiopathologic factors such as head and neck radiation, surgically treated head and neck tumours, connective tissue diseases, facial burns, reconstructive lip surgeries and the most common factor oral submucous fibrosis. Microstomia in patients with trismus induced by submucous fibrosis, the procedure may be even more difficult to carry out because of reduced tissue resiliency and obliteration of vestibular spaces, tongue rigidity and the decreased oral opening. It is difficult to apply conventional clinical procedures to construct dentures for such patients, as it is difficult to insert or remove the custom trays, denture bases and the final prosthesis as one piece. So sectioning of the tray is necessary, so that the trays can be inserted and removed in sections. The main problem encountered during these procedures is reorientation of the tray back in position. This article presents a simple technique for easy handling of the sectioned tray, which is, cost-effective and does not require any special devices or complex locking joints.

Oral submucous fibrosis (OSF) is a chronic inflammatory disease of the oral soft tissues with progressive juxta-epithelial fibrosis resulting in increasing difficulty in chewing, swallowing, speaking and mouth opening, often associated with burning sensation inside oral cavity that is aggravated on exposure to spicy food. It is a common condition in India, related to the habit of chewing betel nut (Areca catechu). It is considered to be associated with genetic predisposition, infectious and viral agents, carcinogens, nutritional and immunologic factors. The visible blanching of the oral mucosa with a marble-like appearance in patients of OSF is attributed to inflammation, followed by hypovascularity and fibrosis that may be associated with small vesicles and mucosal erosions.1 Moreover; betel nut chewing is a major risk factor for the development of malignancies of the gastrointestinal tract. Concomitant to tobacco chewing, smoking and consumption of alcohol, it increases the risk of oropharyngeal malignancies.27% to 13% lesions of OSF can transform into oral cancer, especially squamous-cell carcinoma3.

Class II div I malocclusion is most prevalent type of malocclusion encountered in India. Its management frequently involves use of myofunctional appliance in growing patient. There are several any approaches for crowded mandibular anterior teeth which are currently employed: distal movement of posterior teeth, lateral movement of canines, labial movement of incisors, interproximal enamel reduction, removal of premolars, removal of one or two incisors, and various combinations of the above. Selecting the best treatment is often difficult, and all guidelines do not apply to every case. A case report is presented one mandibular incisor extraction treatment of a 17 year-old male with a Class II malocclusion that shows a significant mandibular arch length deficiency and mandibular tooth-size excess. After careful evaluation of the degree of mandibular anterior dental crowding, existing mandibular tooth-size excess, and the dental midline discrepancy a treatment protocol of extraction of one mandibular incisor was decided upon.

A stem cell is capable of propagating and generating additional stem cells, while some of its progeny can differentiate and commit to maturation along multiple lineages giving rise to a range of specialized cell types. A stem cell can divide asymmetrically, in which one of the two daughter cells retains the stem cell characteristics while the other is destined for specialization under specific conditions. There are two broad categories of stem cells:embryonic and adult stem cells. Embryonic stem cells are derived from embryos. Adult stem cells of dental origin present themselves to be the best contenders for this process. Some of the common sources are dental pulp stem cells (DPSCs), periodontal ligament stem cells (PDLSCs), dental follicle progenitor stem cells (DFPCs) and stem cells from apical papilla (SCAPs)

Dentine hypersensitivity (DH) is a painful response of the tooth to different stimuli. Aims: To evaluate the immediate efficacy in the reduction of dentine hypersensitivity (DH) when applying 810nm diode laser (DL), and commercial toothpaste containing Arginine bicarbonate. Methods and Material: Sixty periodontal maintenance patients of both sexes, with a DH≥2 on the verbal rating scale (VRS) in one or more teeth, were randomly allocated into three equal groups: 20 patients received DL and placebo gel (group A); 20 patients were tested with placebo laser and a commercial toothpaste containing arginine bicarbonate (group B) and the remaining 20 received a placebo laser and placebo gel (group C). The DH was evaluated at the start of the study, 15 and 30 minutes after the laser application, and on days 2 and 7 by a blind examiner. Statistical analysis used: Mann Whitney U Static; Kruskal- Wallis statistic Results: Group A shows greater effectiveness in reducing dentine hypersensitivity. Conclusions: There is definite clinical improvement in DH patients when diode laser is used, with slight clinical advantage over arginine bicarbonate.

AIM: To discuss the effectiveness, drawbacks and the future perspective of Full mouth Disinfection technique BACKGROUND: Periodontal destruction occurs when the microbial load within the periodontal pocket surpasses the local and systemic host defence mechanisms. Reduction of this load by prevention of recolonization is achieved by disinfection of the intraoral sites. Hence full mouth disinfection TECHNIQUE: Scaling and rootplaningof all teeth within 24 hours was carried out in order to reduce the number of remaining bacteria in the pockets, they are irrigated three times within 10 min with a 1% chlorhexidine gel and also chlorhexidine spray was used for disinfection. CLINICAL SIGNIFICANCE: Antimicrobial effect of the irrigation and rinsing product i.echlorhexidine provides disinfection for the sites of the oral niches which is the basis of the reduction of the micro flora. CONCLUSION: Full-mouth disinfection might suppress therisk of cross-contamination between treated anduntreated sites as well as other ecologic habitats.A full-mouthapproach to treatment might prove more beneficialthan conventional nonsurgical treatment, which isperformed in a quadrant-wise or sextant-wise manner.

It is thought-provoking that abuse and infanticide is existing over the centuries, but it is only recently due to change in social values have led to the identification of child abuse as a prevalent medico-social problem nationally and internationally.

Oligodontia is the congenital agenesis of 6 or more permanent teeth,excluding third molars.The prevalence of oligodontia has been reported as 0.3% and affects females more often than males,with a gender ratio of 3:2.Although oligodontia can occur over with 60 different syndromes,these anomalies can occur without any syndrome or systemic disease.All mutations of PAX9 identified to date have been associated with nonsyndromic form.This case report describes a multidisciplinary treatment approach toward a female patient 16 years of age presenting with familial nonsyndromic oligodontia having paternal family trait and absence of 28 permanent teeth.The treatment plan considered for the patient included extraction of deciduous teeth with resorbed roots and poor prognosis. Remaining deciduous teeth which were affected by secondary caries were restored by endodontic therapy.Finally rehabilitation was done by removable partial denture for maxillary arch and overdenture for mandibular arch.It is emphasized that conventional prosthetic treatment can lead to a satisfactory result.

Sarcoidosis is a multi-system granulomatous disease of unknown etiology, most commonly affecting adults and at times children. The disease characteristically causes bilateral hilar Lymphadenopathy and granulomatous, pulmonary, skin or ocular lesions. Sarcoidosis was first described in 1877 by the British surgeon dermatologist, Jonathan Hutchinson. Caesar Boeck of Oslo, Norway, fully described sarcoidosis in 1899, hence also known as Boecks sarcoidosis. Venkatraman et al 1995 reported a case of papillary psoriasis; they discussed a 50 years old man who had non-tender purplish plaques on both hands and feet. Ten years later, the classic histological pattern of the disorder was described, and it was pointed out that the syndrome of sarcoidosis involved not only the skin but also the lymph nodes, the mucous involves and the lungs. At that time the disorder received its presently accepted name Sarcoidosis. Later sarcoidosis was more formally defined as a disease characterized by the presence of non-caseating epithelioid granulomas, proceeding either to resolution or to conversion into featureless hyaline connective tissue. Measures for infection control stresses the importance of using gloves and masks. But this has increased risk of danger due to latex exposure either by contact with skin surface or inhaled airborne particles amalgamated from gloves. Reaction ranges from mild contact dermatitis to potentially life threatening hypersensitivity. Rate of allergy shows increase with increased and repeated exposure to allergens arising from orthodontic products. The aim of this article is to find the sources of latex exposure and life threatening reactions arising from it and necessary corrective measures that need to be taken.

Objective: To evaluate the in vivo effect of diode laser when used alone or in combination with various irrigants on root canal microbes Methodology: A total of 25 patients in age group of 15-45 of distal canal of Permanent molars were selected from patients who had visited to the Department of Conservative and Endodontic Dentistry, Chandra Dental College and Hospital, Barabanki. Healthy and cooperative patients were selected for this study. They were subsequently divided into five groups comprising of 5 patients each - In Group I, only diode laser was used. In Group II, diode laser was used in combination with 5% sodium hypochlorite solution. In Group III, diode laser was used in combination with 2% Chlorhexidine and in Group IV, diode laser was used in combination with saline whereas Group V was treated with a combination of Sodium hypochlorite, 2% Chlorhexidine and Saline solution. Before treatment and after treatment microbial assessment (pathogen, colony count and aerobe/anaerobe typing) was done. Data was analyzed using SPSS 21.0 software. Kruskal-Wallis and Wilcoxon signed rank tests were used. Results and Conclusion: Positivity rate for aerobes ranged from 60% to 100% and for anaerobes 60% to 80% in study groups. Staphyloccus aureus, Coagulase negative Staphylococcus, Streptococcus and Bacillus spp. were the aerobic isolates. Enterococcus faecalis and Pseudomonas intermedia were the anaerobic isolates. Mean reduction in aerobic colony count ranged from 73.92±6.36% (Group I) to 83.88±10.85% (Group V). Mean reduction in aerobic colony count ranged from 70.43±24.08% (Group V) to 95.98±6.97% (Group II). Statistically there was no signfiicant difference among the groups. On evaluating the extent of reduction in microbial count, the only significant reduction was observed in Groups IV and V for anaerobes (p=0.048). This preliminary assessment showed that diode laser in combination with saline was comparable to a multimodal modality of NaOCl, 2%CHX and Saline combination.

Postoperative oedema and trismus after the third molar surgery are commonly encountered problems by the Oral and Maxillofacial Health Care Workers. Corticosteroids have two main pharmacological actions. The 1st one is immunosuppressive action and the 2nd one is anti-inflammatory action. Corticosteroids are given in the case of mandibular third molar surgery due to their antiinflammatory action.Intramuscular, intravenous or sub mucosal injection of dexamethasone appears to be effective in the prevention of postoperative oedema. It can be given either as preoperative, perioperative or postoperative injection.

This case report describes extraction of a hopeless tooth s followed by immediate implant placement of a dental implant in the freshly extracted socket. The implant placement into the fresh extraction socket reduces the treatment time, cost, preserves hard and soft tissue architecture and an increased comfort of the patient. The prosthetic treatment of seriously damaged, endodontically treated teeth often require an endodontic post as an additional retentive element for core build-up prior to placement of the crown. For post and core placement there should be atleast 2mm of sound tooth structure to provide ferrule effect. Periodontal crown lengthening or orthodontic tooth eruption is carried out if the sound tooth structure is not sufficient for ferrule design. These posts can be custom made or pre-fabricated. Pre-fabricated composite post systems are replacing metal post systems because of their superior properties like increased retention, no corrosion, and good esthetics. This case report demonstrates the restoration of badly mutilated endodontically treated tooth by fiber reinforced post after periodontal crown lengthening.

Oral cancer is the 8th most frequent cancer in the world among males and 14th among females. The most common type of oral cancer is squamous cell carcinoma (SCC). SCC is a malignant neoplasm of epithelial origin. In India, buccal mucosa and gingivobuccal sulcus are frequent sites of oral SCC (OSCC) but in recent times increasing number of tongue SCC have been reported. The etiology of OSCC is multifactorial with both extrinsic and intrinsic factors playing important role. A definite causal association has been found between OSCC and smoking and/or alcohol consumption. But over time, OSCC case reports in a non-smoking non-drinking (NSND) population are also emerging. This case report presents a case of OSCC induced by chronic mechanical irritation with no deleterious habit history.

Patient with generalized anterior spacing (Natural diastema) present problems, as the increased mesio-distal space makes it difficult to give normal fixed prosthesis with rigid connector. Uniform diastema present before extraction may result in excessive width of the pontic space and wider pontic can generate undue forces over retainers which may leads to failure of prosthesis and also it's very unesthetic to look at in the esthetic zone. This is a challenging clinical situation for the prosthodontist if an implant-supported prosthesis is not possible or not selected as a treatment option; loop connector fixed partial denture may be the best solution to maintain the spacing and esthetics. This case report has been describing the procedure of replacing missing anterior teeth with loop connector fixed partial prosthesis while maintaining diastema [1].

Introduction: Organic solvents like Chloroform and Xylol are commonly used to facilitate removal by softening gutta-percha with sealer whereas Endosolv R is an endodontic solvent used to soften resin based sealers. Two commonly used sealers in Endodontics are MTA Fillapex (Mineral trioxide aggregate based) and AH-Plus. Aim:To compare the ability of three commonly used endodontic solvents to soften gutta-percha and resin based sealers (two) to allow for the re-establishment of apical patency. Material and Methods: Ninety extracted human mandibular premolar teeth were divided into two groups after instrumentation (n = 45) based on the sealer used; Group A was obturated with guttapercha and AH Plus whereas MTA Fillapex was used in Group B. Both groups were divided into three subgroups (n = 15) based on the solvent used. Each subgroup was exposed to 1 of the following solvents: Chloroform, EndosolvE, EndosolvR. Results: Patency was re-established in 100% of the teeth in Group B when tested with Chloroform or Endosolv R. Chloroform was 100% successful in establishing patency in both groups. The Chi-square and Fisher's exact test indicated a statistically significant difference between Endosolv E and the other tested solvents for Group B. Chloroform was found significantly better than Endosolv E in Group B and better than both Endosolv E and Endosolv R in Group A. Conclusion:Chloroform and Endosolv R softened MTA Fillapex sufficiently to aid in re-establishing apical patency whereas Endosolv R considerably failed to establish patency through AH Plus during endodontic retreatment.
