Introduction: The aim of this study was to determine the incidence of dentinal cracks in the mesial root of maxillary molar during canal preparation using Neoniti system in different torque settings. Materials &Methods: In this in-vitro study, 60 maxillary molars extracted for various reasons were selected. The teeth were divided into 4 groups: one group(n=15) without preparation was considered as a control group (unprepared control group), the other 3 groups prepared with rotary neoniti system: group with standard torque (1.5 N/CM2)(n=15), group with high torque (2N/CM2)(n=15), and group with low torque (1N /CM2)(n=15). After a canal preparation procedure, the teeth were horizontally sectioned at 3, 6 and 9 mm from the apex. All sections were examined for determining the presence of cracks using a stereomicroscope. Data were analyzed using chi-square test. Results: There was no crack in the control group. The number of cracks was significantly higher in the high-torque group (80%) than standard- and low-torque groups (20%, 26.7%, respectively) (p<0.001). However, no significant difference was found between the standard-torque group and low-torque group (p<0.001). Conclusion: According to this study result, to avoid crack formation in higher torques using motors with torque control option is suggested.

Introduction: Ability to reconstruct the papilla in anterior maxilla is important aspect of perio-plastic surgery. In most articles, connective tissue is used with different designs of incisions. The aim of this study was to use sub-epithelial connective tissue graft (SCTG) with two types of incisions called papilla preservation and semilunar. Materials & Methods: This basic randomized clinical study was performed on 10 sites in two patients. The patients were selected through inclusion and exclusion criteria. Papilla preservation and semilunar techniques were performed on four and six sites, respectively in the anterior maxilla. In both techniques SCTG was gained from palate .The apico-coronal and mesiodistal changes of the dark triangles were measured after 3 and 6 months. Landry(Healing) index was measured after 14 days and one month,Visual Analogue Scale (Esthetic) index was estimated in 3 and 6 month after surgery and Visual Analogue Scale (VAS ) index was analysed as well . Data were analysed using SPSS. Mann- Whitney, Wilcoxon and Paired t- Test were measured. Results: Mean±SD of mesiodistal distance in the time of surgery was 2.00±0.000 in semilunar and 2.1±0.629 in papilla preservation technique whereas after 3 months, it was 1.33±0.016 and 1.37±0.478 for semilunar and papilla preservation, respectively and after 6 month was 1.00±0.000 for semilunar and 1.25±0.500 for papilla preservation. Mean±SD of apicocornal changes by semilunar incision in the time of surgery ,3 month after and 6 months later was 2.67±0.516 ,2.25±0.612and1.91±0.204 whereas by papilla preservation was 2.50±0.577,2.25±0.500 and 2±0.000, respectively. Conclusion: Both techniques had positive effect on papilla reconstruction and the outcome was the same in both groups.

Introduction: Root treatment will not be successful, without a proper root canal irrigation with less disadvantages and antibacterial effect. The aim of this study was to compare antimicrobial effect of cedar extract and 2.5%NaOCl on E. faecalis. Materials & Methods: In disk diffusion test, a standard suspension of E. faecalis (ATCC 29212) was cultured on plate and different concentrations (0.05, 0.15, 0.25, 0.35, 0.45 g/ml) of methanolic or hydro-alcoholic extracts, 2.5% NaOCl and physiologic serum (as negative control) were infused on paper disks. The inhibition zone measured after 48 h. In microdilution test, serial dilution of methanolic and hydro-alcoholic extracts, 2.5% NaOCl and physiologic serum in 1:2 proportion was performed in Brain Heart Infusion (BHI) culture medium. Then, standard suspension of E. faecalis was added to each well of micro plate. Data were analyzed using ANOVA. Results: Hydro-alcoholic and methanolic extracts had antibacterial effect on E. faecalis. Inhibition zone of 2.5% NaOCl was significantly higher than that of other extracts (p<0.001). In microdilution test, E. faecalis bacterium was sensitive to both hydro-alcoholic and methanolic extracts but it was more sensitive to 2.5% NaOCl. Conclusion: Totally, 2.5% NaOCl had the highest antibacterial effect on E.faecalis followed by hydro-alcoholic and methanolic extracts. NaOCl is an effective irrigant in root treatment until the studies like this can find a good alternative for it.

Introduction: The aim of this study was to recognize and investigate the expectations of patients and their parents from orthodontic treatment in order to increase the satisfaction from treatment outcome and enhance the patients’ cooperation in Babol in 2017. Materials & Methods: Totally, 200 people (100 patients aged 12-18 with one of their parents) who were attending for their first orthodontic treatment session participated in this cross-sectional study. Participants completed a self-administered questionnaire which was translated by Forward-Backward method from English to Persian language. Data were analyzed using SPSS 22 through descriptive statistics and t-tests. Results: The most important expectations of patients and their parents from the first appointment of orthodontic treatment were check -up, diagnosis, discussion about treatment, and oral hygiene checking. Patients’ expectations from first appointment were higher than their parents in “brace being fitted” (p=0.001), lower in “have a discussion about treatment plan” (p=0.006), “have x-rays” (p=0.003), and “have oral hygiene checked” (p=0.03). The highest expectation of patients as well as their parents from the type of orthodontic treatment was fixed braces. The main expectation of patients and parents from orthodontic treatment was the demand for straightening teeth and improving aesthetics. Conclusion: Parents than patients had more reasonable expectations from the first appointment of orthodontic treatment. Parents had higher expectations from orthodontic treatment benefits. Age and gender did not have significant effect on the type and level of expectations of parents and patients.

Introduction: Although recent studies show the association of eosinophils with different tumors including oral malignancies, the role of eosinophils in premalignant conditions of the oral cavity is not known precisely. Therefore, this study aimed to determine the number of eosinophils in premalignant and malignant conditions of the oral cavity and its role in progression to malignancy. Materials &Methods: This cross-sectional study was done on 50 samples with oral squamous cell carcinoma (OSCC), 18 with verrucous carcinoma, 13 with erosive lichen planus, and 10 with leukoplakia with mild dysplasia after confirmation of histopathological diagnosis. The samples were stained with Sirius Red. The total numbers of eosinophils in 10 areas were counted at a magnification of 400x (eosinophil/10HPF). Data are analyzed with Kruskal-Wallis test, Mann-Whitney test, and Spearman’s correlation test. Results: The mean number of eosinophils in OSCC, verrucous carcinoma, erosive lichen planus, and leukoplakia with mild dysplasia was 90.26, 59.72, 11.08 and 0.0 eosinophil/10HPF respectively. The mean number of eosinophils was significantly higher in malignant (OSCC and verrucous carcinoma) than premalignant (erosive lichen planus and leukoplakia with mild dysplasia) lesions of the oral cavity (P=0.0001). No significant correlation was found between the number of eosinophils with histopathologic grade, gender, and age in OSCC samples. Conclusion: The greater number of eosinophils is seen in malignant than premalignant lesions of the oral cavity. It is recommended to consider the high number of eosinophils in premalignant lesions as an indicator for a more potential of preliminary oral lesions to undergo malignant transformation.

Introduction: Incisive foramen in the maxilla is the oral terminus of the nasopalatine canal. This region is important in implantology, Therefore, the aim of this study was to investigate the neurovascular anatomical variations in the maxillary anterior region, observed on cone beam computed tomography (CBCT) images. Materials &Methods: In this cross-sectional study, maxillary CBCT images from 200 patients (100 men, 100 women, average age 45.61±11.01) were evaluated in sagittal, axial and coronal sections and the following parameters were investigated: incisive canal diameter, shapes of canal, presence of accessory canal in the anterior palate and their location, thickness of buccal bone in the sections containing canal, patient's dental and periodontal status in terms of bone loss. Data were collected and analyzed using SPSS, V20. P≤0.05 was considered significant. Results: In sagittal view, the mean diameter of the canal was 2.36±0.69mm.Age and gender did not significantly influence the diameter. Morphological analysis of canal showed that 25% of them were hourglass-like,23.5% conical,22%funnel,27.5% cylindrical, 2% tree branch. Accessory canals were observed in 58 patients (29%) specially near to the canine region. The thickness of buccal bone plate was less in complete edentulous patients than others and also there was a significant relationship between bone loss and canal size in the hourglass and cylindrical shapes. Conclusion: In this study, the prevalence of accessory canals was relatively significant, so it seems essential to carry out cone beam computed tomography in this area for determining the canal morphology and dimensions before implant placement.

Introduction: Increased consumption of energy drinks has raised concerns about their effects on dental restorations. This study assessed the effects of two energy drinks on the surface microhardness of methacrylate and silorane-based composites after 1-week and 1-month periods. Materials & Methods: In this in-vitro study, 90 cubic samples were prepared from Filtek P90, Filtek Z250 and Filtek Z350 XT composite resins. Vickers hardness test was performed to measure the baseline surface microhardness for each specimen. Ten randomly selected samples from each composite material were then immersed in one of the two sports drinks (Red Bull and Hype) or artificial saliva (control). Surface microhardness was re-evaluated after 1 week and 1 month of immersion. The data were evaluated using ANOVA via post-hoc Tukey tests and repeated measure test (α=0.05). Results: Surface microhardness of all composites were significantly decreased in energy drinks in both evaluation periods (P<0.001). In artificial saliva, microhardness was significantly increased after 1 week and decreased after 1 month of immersion (P< 0.001). After 1 month, the lowest microhardness changes were observed in Filtek Z350 XT composite. (18% and 14% reduction in Hype and Redbull respectively). Differences between energy drinks were significant for Z350 XT composite only after 1 week (P=0.01) and for Z250 composite after 1 week and 1 month (P=0.020 and P< 0.001 respectively). Conclusion: Hype and Red Bull energy drinks can affect the surface hardness of composite resins depending on their characteristics and exposure time.

Introduction: Oral cancer is the most common malignancy in head and neck region. Primary diagnosis is critical and low knowledge of dentists can lead to improper or delayed diagnosis. The aim of this study was to evaluate the knowledge of general dentists regarding oral cancer and the complications of its treatment in Babol, Iran. Materials &Methods: This cross-sectional study was performed on all general dentists working in Babol. Seventy-six dentists entered into the study and filled out the questionnaire. Data were analyzed by SPSS software. Results: Mean score of knowledge in the group with more than 21-years-experience was significantly higher than the group with less than 10-years-experience (p=0.017). There was a positive correlation between total knowledge and years of experience (p=0.001). There was no significant relationship between other criteria with total score of knowledge (p.0.05). Conclusion: It seems that knowledge the babol dentists’ regarding oral cancer and cancer treatment complications is not sufficient.

Introduction: Resin modified glass-ionomer cement (RMGI) is prepared by manual mixing of powder and liquid. Different mixing ratios influence on the RMGI properties. The aim was to compare the effect of different mixing ratios on the microleakage of RMGI. Materials &Methods: In this in vitro study, 60 Class V cavities (3×2×1.5 mm) with the gingival margin of 1 mm apical to the cement-enamel junction were prepared on the buccal and lingual surfaces of 30 sound premolars. The teeth were randomly divided into 6 groups. Group 1: The manufacturer’s recommended ratio, without conditioning; Group2: The manufacturer’s recommended ratio with conditioning; Group 3: 20% lower than the manufacturer’s ratio without conditioning; Group4: 20% lower than the manufacturer’s ratio with conditioning; Group 5: 20% higher than the manufacturer’s ratio without conditioning; Group6: 20% higher than the manufacturer’s ratio with conditioning. After thermocycling, the microleakage was evaluated using silver nitrate staining. The teeth were cut into two mesial and distal halves, and the microleakage at occlusal and gingival margins was recorded based on a 0‒3 scoring system under a stereomicroscope. Data were analyzed using Kruskal-Wallis and Mann-Whitney tests with significance level at P<0.05. Results: The maximum microleakage at gingival margins was recorded for group 4, which was significantly higher than that of group 2 and 6 (P=0.043 and P=0.043, respectively). No significant differences were observed in the microleakage between occlusal and gingival margins. Conclusion: A 20% reduction in P/L ratio of RMGI increases the gingival microleakage when surface conditioning was applied.

Introduction: Cervical root resorption is one of the most important complications of intra coronal bleaching. A way of preventing this type of resorption is using a coronal barrier under the bleaching materials. The aim of this study was to compare the sealing ability of glass ionomer cement and Pro Root Mineral Trioxide Aggregate (MTA) as a coronal barrier in intra coronal bleaching. Materials &Methods: In this study, 40 single-root maxillary anterior teeth were endodontically prepared and divided into two experimental groups (n= 15) and two positive and negative control groups (n=5). In the experimental groups, gutta percha was removed up to 3 mm below the cemento enamel junction (CEJ).RMGI and MTA were placed over gutta percha up to the level of CEJ. After a 24-hour incubation period, the bleaching agent (a mixture of sodium perborate and 30% hydrogen peroxide) was placed in the access cavities. The bleaching agents were replaced every 3 days over 9 days. Then, the access cavity was filled with 2% methylene blue for 48 hours. All samples were longitudinally sectioned and the dye penetration range was evaluated using a stereomicroscope. Data were statistically analyzed using Kruskal-Wallis and Mann–Whitney tests (=α0.05). Results: Leakage mean indicated that there was a significant difference between these two groups and leakage was less in ProRoot than glass ionomer. Conclusion: It seems that the MTA can provide a better coronal seal during the bleaching.

Introduction: Obesity as a pervasive phenomenon in recent years has the risky consequences on public and oral health and endangers the teeth especially periodontal tissues. This aim of this study was to assess the relationship of oral health (teeth and periodontal tissue) with obesity and anthropometric measures such as waist circumference (WC) and body mass index (BMI). Materials &Methods: This cross sectional study was conducted on 180 subjects in 3 groups of normal weight, over-weight and obese. Periodontal pocket depth (PPD), bleeding on probing (BOP) and Community Periodontal Index (CPI) were recorded. Multivariate logistic regression was also applied after adjusting for the confounding factors. Results: Of 180 subjects, 54, 68 and 58 cases were normal, overweight and obese. 75 and 105 participants were male and female, respectively. Generally, a pocket depth was increased 1.394 times with one unit increase of BMI (OR: 1.394, 95% CI: 0.936-2.077). Dental caries index enhanced to 1.036 with one unit increase of waist circumference (WC) (OR: 1.036, 95% CI: 1.001-1.071). One centimeter rise of WC increased CPI up to 0.625 times (OR: 1.122, 95% CI: 0.053-0.078). Conclusion: Obesity even in the absence of underlying systemic diseases can cause the potential risk in oral health.

Plexiform schwannoma is a type of schwannoma that can be occured at any age and any site of the oral cavity. In the gross and histopathological features, it shows plexiform or multinodular growth pattern. This variant of schwannoma is sometimes associated with neurofibromatosis type II or schwannomatosis. Histopathologically, plexiform schwannoma is composed of schwann cells that show Anthony A growth pattern. Mitosis are rare or absent. This study reports a case of a healthy 34 year old female with swelling in floor of her mouth; according to the clinical and histopathological findings, the diagnosis was Plexiform schwannoma.

Introduction: Evaluation is a critical issue to achieve the goals of academic education. The aim of this study was to evaluate the achievement level of educational objectives in Babol dental school using the CIPP (content, input, process, and product) model based on the point of view of students. Materials &Methods: This cross-sectional study was performed using a researcher made questionnaire based on CIPP model for three educational groups of pediatrics, orthodontics and restorative dentistry among dental students accepted in 2008 and 2009. Total scores were calculated for each field and categorized as undesirable, relatively desirable and desirable with scores below 50, 51-70 and 71-100, respectively. Statistical analysis was performed using ANONA, T-test and Tukey HSD tests and p<0.05 was considered significant. Results: The mean scores were desirable in all groups. Mean scores allocated to the content, input, process and product areas were not significantly different in the pediatrics, orthodontics and restorative dentistry groups. Conclusion: Based on the student’s point of view, the pediatrics, orthodontics and restorative dentistry departments of Babol dental school were successful in achieving educational goals.

Introduction: Differentiation of fibroblasts to myofibroblasts is an initial and very important event in tumor genesis. Myofibroblasts produce proteinases that stimulate invasion in cancers. Due to the more malignant potential of oral squamous cell carcinoma (OSCC) compared to cutaneous squamous cell carcinoma (CSCC) , the aim of the present study was to compare myofibroblasts between OSCC and CSCC to understand whether myofibroblasts can help more malignant potential of OSCC compared to CSCC or not. Materials &Methods: This cross-sectional study included 40 cases of OSCC and CSCC and 20 cases of normal skin and normal oral mucosa. Then, 4-micron sections of paraffin-embedded tissue blocks of studied groups were stained immunohistochemically with α-SMA antibody. Mean percentage of myofibroblasts was calculated in invasive fronts of OSCCs with CSCCs and also in normal samples and staining intensity of cells for α-SMA marker and distribution pattern of myofibroblasts were determined. Results: The differences of average percentage of myofibroblasts in OSCC and CSCC compared to normal groups were significant (Pvalue= 0.007 and Pvalue=0.003 respectively), but when we compared OSCCs and CSCCs, the difference was not significant. Also, there were no significant differences between OSCC and CSCC with regard to staining intensity and pattern. Conclusion: Different biologic behavior of OSCC compared to CSCC doesn’t depend on myofibroblasts and other factors can be involved.

Introduction: Alpha amylase is the most abundant enzyme of parotid gland. This enzyme starts digestion of carbohydrates in the mouth. A pair of parotid glands is located in front of ears. Several studies have been conducted on the effects of mobile phones on the parotid gland. The aim of this study was to investigate the effects of mobile phones on the activity of the amylase enzyme. Materials &Methods: Totally, 251 men and women aged 18 to 55 years, who only used Hamrahe Aval's operator were included. Parotid salivary samples of each individual were bilaterally collected using capillary tube placed in the vicinity of Stensen's duct. Bilateral amylase activity of each individual was evaluated with spectrophotometric method in the laboratory considering dominant sides of phone conversation and chewing. Data were analyzed using Two-way ANOVA, χ2, T test and SPSS software. Results: The relationship between the dominant phone conversation side and parotid amylase activity was not statistically significant but the correlation between prevailing chewing side and amylase activity was statistically significant (p=0.001). Conclusion: Handheld mobile phone was not effective on parotid amylase enzyme activity whereas chewing was effective on parotid amylase enzyme activity.

Introduction: The present study was performed to evaluate the effects of light-curing modes and shades of methacrylate and silorane-based composite resins on temperature rise during polymerization of composite resins. Materials &Methods: In this in vitro study, ninety composite samples with two types of composite resin - methacrylate-based and a low-shrinkage silorane-based were prepared, in 3 groups (30 samples in each group), Group 1: Filtek Z250 (shade A3), Group 2: Filtek Z250 (shade B2) and Group 3: Filtek P90 (shade A3). To evaluate temperature changes, 15 samples and the other half were light-cured using the standard and soft start curing modes for 40 seconds, respectively. A k-type thermocouple was used to recorded temperature changes. Results: Temperature rise in the silorane-based composite resin group (group 3) was higher than that in the methacrylate-based one (group 1) (P<0.001). Temperature rise with the darker shade (group 1) was higher than that with lighter shade (group 2) (P<0.001). Standard curing resulted in more temperature rise compared to soft start curing; however, the difference was only significant in group 2 (P<0.001). In all groups, temperature increased with an increase in curing time and the temperature rise pattern during the first 10 seconds with the soft start curing was not consistent with the standard curing (in group 2 and 3); the temperature rise pattern was milder than the soft start curing (P<0.001). Conclusion: Factors such as composite resin type, shade and curing mode affected temperature changes in composite resins evaluated in the present study.

Introduction: Internal bleaching is a treatment option for wightening endodontically treated discolored teeth. Cervical resorption is one of the side effects of this method. The aim of this study was to compare the sealing ability of resin composite and light-cured resin modified glass ionomer (RMGI) as intra-orifice barriers in internal bleaching. Materials &Methods: In this study, 34 single-canal anterior teeth were used. All samples were endodontically prepared and divided into two experimental groups (n=12) and two control groups (n=5). In the experimental groups, Gutta-percha was removed up to 3 mm below the cemento enamel junction (CEJ). RMGI and composite resin was placed over gutta-percha in the experimental groups up to the level of CEJ. After 24-hours incubation period, the bleaching agent (a mixture of sodium perborate and 30% hydrogen peroxide) was placed in the access cavities. The bleaching agents were replaced every 3 days over 9 days. Then, the access cavity was filled with 2% methylene blue for 48 hours. All samples were longitudinally sectioned and the dye penetration range was evaluated using stereomicroscope. Data was statistically analyzed by using T-student test and variance analysis. Results: The microleakage in RMGI group was 0.945mm and in composite resin group was 0.641mm. Statistically, no significant difference was observed in microleakage between the experimental groups (p=0.121). Conclusion: Both materials can be applied as the intra-orifice barriers for internal bleaching.

Introduction: Working length determination is important in successful endodontic treatment and retreatment. This study evaluated the accuracy of two electronic apex locators Root ZX and Raypex®6 (EALs) in determining the electronic working length (EWL) of the root canals in endodontic treatment and retreatment. Materials &Methods: Access cavities were prepared on forty extracted, single-rooted human teeth and the actual working length (AWL) of the canals was determined. In the first phase of the study, primary EWL of un-instrumented teeth was measured and compared between two EALs. In phase II, all of the teeth were pre-flared and divided into the control (n=10) and the retreatment groups (n=30). Canals in the retreatment group were obturated by the lateral condensation technique using Gutta percha and sealer. After 15 days, gutta-percha was removed, and then the secondary EWL was recorded and compared between the two devices, in treatment and retreatment groups. Data were analysed by paired t-test and t-test. Results: Significant differences were found between both EALs in treatment and retreatment phases of the study (p<0.001). Both EALs showed increased accuracy in retreatment group (p<0.001). However, no statistically significant differences were found between the control and retreatment groups in the second phase of the study for Root ZX (p=0.929), and Raypex®6 (p=0.937). Conclusion: Accuracy of the two EALs was similar and acceptable. EWLs determined by Root ZX were closer to the AWL. The EWL determination after pre-flaring improved the accuracy of EALs and root canal obturation remnant materials did not have any clear effect on the accuracy of these EALs.

Introduction: Bleaching of discolored tooth may affect the tooth/composite interface. The aim of this study was to evaluate the effect of different concentrations of carbamide peroxide (CP) on the marginal seal of composite restorations bonded with a self-etch adhesive. Materials &Methods: In this experimental study Class V cavities were prepared on the buccal and lingual surfaces of 24 intact extracted human molar teeth with gingival margins in dentin and occlusal margins in enamel. The cavities were restored using the adhesive system Clearfil SE Bond and the composite Filtek Z250. Finally, they were randomly divided into four groups (1 control and 3 bleached groups). The control group was kept for two weeks in distilled water at 37° C. The bleached groups were bleached with 10%, 22% and 35% CP gel for 8 hours a day for 14 days. After that, the samples were immersed in 0.5% fuchsin solution and sectioned. Then gingival microleakage was graded. Data was analyzed using Kruskal Wallis and Mann Whitney U test (P≤0.05). Results: Microleakage was observed in the gingival wall of all groups. There was a significant difference between the control group and the bleached groups. Microleakage in the 22% bleached group was less than the other concentrations, but this difference was not significant. Conclusion: Bleaching with different concentrations of CP can increase the microleakage of the gingival wall of Composite restorations bonded with clearfil SE Bond self-etch adhesive and does not recommend.

Introduction: Periodontal disease is a common chronic inflammatory disease, considered as a primary cause of tooth loss due to the destruction of periodontal tissues. Various studies have shown that vitamin D has anti-inflammatory and anti-microbial properties and plays a significant role in musculoskeletal health. There is strong evidence that vitamin D deficiency may increase the likelihood of osteoporosis and osteopenia as well as chronic inflammatory diseases such as periodontitis. Materials &Methods: This case–control study included 30 females with moderate or severe chronic periodontitis (CP) and 30 periodontally healthy females. The mean age of the CP group and the control group was 34 years. Indices of Probing depth (PD), clinical attachment level (CAL) and bleeding index (BI) as well as the serum level of vitamin D (1,25(OH)2D) were measured in both groups. The statistical analyses including t-tests, Pearson correlation coefficient and odd ratio. Results: There was a significant indirect relationship between the serum level of vitamin D and the periodontal indices (p<0.05). The odd ratio (OR) measures showed that subjects with vitamin D dose of less than 10 ng/ml were 5.6 times more likely to have periodontitis compared to those with a normal dose of vitamin D ( p=0.03 ). Moreover, subjects with vitamin D dose of 10-29 ng/ml were about 1.46 times at a higher risk to develop periodontitis than those with sufficient dose of vitamin D (p>0.05). Conclusion: It is recommended that serum levels of vitamin D in patients with chronic periodontitis be measured and then if necessary, treatment should begin.
