Meccaniche posturali del Lumbo-Pelvic-Hip Complex in atleti con Low Back Pain. Implicazioni per un corretto approccio riabilitativo – funzionale.

Backgroung. La stabilità del Lumbo-Pelvic-Hip Complex è una componente essenziale per migliorare le performance atletiche e decrementare il rischio di infortuni nello sportivo. Obiettivo. Studiare le dinamiche posturali del complesso pelvico e correlarle con il Low Back Pain per verificare, successivamente la validità degli esercizi eseguiti su superfici instabili (Swiss Ball) ed in sospensione (TRX ) sulla stabilità del complesso Lumbo-Pelvic-Hip Complex. Si è cercato anche di identificare i migliori esercizi, per pazienti affetti da Low Back Pain. Metodo. E’ stata eseguita una revisione retroattiva della letteratura. Abbiamo effettuato la ricerca su MEDLINE, EMBASE, PUBMED e Biblioteca Cochrane. Sono stati ritenuti idonei, per questo studio, 111 articoli. Risultati. Quadri disfunzionali a carico del Lumbo- Pelvic-Hip Complex si correlano con un Low Back Pain. In particolar modo evidenze scientifiche sottolineano come asimmetriche rotazioni dell’anca si correlano con un Low Back Pain La letteratura scientifica al momento è carente nel riconoscere l'efficacia di esercizi effettuati su superfici instabili e in sospensione per pazienti nel paziente effetto da Low Back Pain. L’esercizio di abduzione dell'anca in Plank su TRX rimane the best exercise per il miglioramento del Trasverso dell’Addome, Interno Obliquo, Retto Addominale, Multifido. Hamstring Curl su TRX rimane, invece, l’esercizio di elezione per una migliore attivazione dei paraspinali. Il 45° Row exercise genera al contrario una minore attivazione EMG dei muscoli della Lumbo-Pelvic-Hip Complex. Esercitazioni sulla swiss ball rappresentano, rispetto alle stesse su piano stabile, una maggiore attivazione EMG dei Muscoli del Complesso Lumbo Pelvico , E’ evidenziato,anche, che esercizi su Swiss Ball, migliorarono il controllo neuro muscolare del complesso Lumbo - Pelvico. Conclusioni. Abbiamo verificato, grazie ai lavori da noi recensiti, che esercizi su superfici gonfiabili e in sospensione possono trovare applicabilità in atleti con low back pain. Questi modelli di training indirizzati ad un miglioramento della stabilità del complesso Lumbo-Pelvic-Hip Complex hanno, rispetto a superfici stabili, una maggiore attivazione muscolare dei muscoli del tronco. L’esercizio di abduzione dell'anca in Plank su TRX è l’esercizio d’elezione consigliato per la stabilità del Complesso Lumbo Pelvico. Rimane, secondo noi però di importanza capitale la personalizzazione della scelta e la progressione didattica degli esercizi che deve essere in relazione al quadro clinico/funzionale/posturale .

A study to examine the effectiveness of sleeper’s stretch on shoulder horizontal adduction and internal rotation range of motion in male cricket bowlers.

Background: Alteration in glenohumeral range of motion including increase posterior shoulder tightness and glenohumeral internal rotation deficit that exceeds the accompanying external rotation gain are suggested contributors to throwing related shoulder injuries such as internal impingement, shoulder dislocation and SLAP lesion. Many studies have been done to evaluate the effects of sleeper stretch in tennis players and baseball pitchers. Yet these contributors have not been identified in cricketers. Purpose: This study examines the effects of sleeper stretch on shoulder horizontal adduction and internal rotation in male cricket bowler. Methodology: 60 male cricket bowlers were selected from Ahmedabad. Glenohumeral joint horizontal adduction and internal rotation ROM was measured in two groups by digital inclinometer on a period of pre and post treatment week 1, week 2, week 3. Sleeper stretch was given to the affected shoulder for 30 sec hold, and 15 second rest is given between each stretch on alternate days, three times in week for 3 consecutive weeks. Results: This study has identified effects of sleeper stretch on glenohumeral internal rotation and horizontal adduction ROM was insignificant. Conclusion: sleeper stretch is highly effective in gaining glenohumeral horizontal adduction and internal rotation ROM in male cricket bowlers.

Effects of active release technique in males as compared to females with postural and psychological cervical muscle spasm: A Quasi experimental study

Active Release Technique is a soft tissue release technique of the soft tissues which is nowadays being commonly practiced in the world as it gains recognition all around. This technique is being introduced in Pakistani community in this study. Further the purpose of application of this technique was to check the difference between gender responses in neck extensor muscle spasm. Bone mineral densities in Asian community are low especially in females which eventually increase stress on muscles resulting in spasm. In this study 15 females and 14 male patients between the age group 20-60 years were selected through simple random sampling making a total of 29 patients. Potential participants were examined and selected through inclusion and exclusion criteria and regional examination of C-spine. 2 questionnaires were used i-e, QVAS and NDI on which subjective measurements were taken. Both groups received heat fermentation for 10 minutes and then were applied with Active Release Technique 3-5 times. This was repeated for 4 sessions on alternate days. Subjective data was collected pre and post treatment on two instruments i-e, Neck Disability Index and Quadruple Visual Analog Scale. The data was recorded and manipulated on SPSS software for analysis. Statistically and clinically significant improvements were seen in both groups throughout the duration of the study with regards to the level of perceived psychological stress, pain and disability. Whereas, female group came up with more improvement. It was seen that ART proved to be better regarding NDI scores as compared to QVAS with a p value of 0.000 and 0.003 respectively. Upon completion of this study, it became evident from subjective data that both treatment groups demonstrated statistically and clinically significant changes. Hence both the groups presented with equal changes as an effect of ART but we have to get more skilled training in this technique. Alteration in scapular position with high intensity training- A study in competitive swimmer

Background. Swimming is a popular sport that encompasses individual of all ages that swim for fitness, recreation and competition. In swimmers, the great number of stroke repetitions and force Generated through the upper extremity leaves the shoulder uniquely vulnerable to Injury. As competitive swimmers usually train 11000-15000 yards/day, using freestyle arm stroke for most of the distance. And with incorrect faulty mechanics leads to repetitive micro trauma at the shoulder joint leads to shoulder pain, muscle weakness and dynamic muscle imbalance that alters the normal scapular positioning and biomechanics where scapula fails to perform its stabilizer role and shoulder function become insufficient. The manual methods such as lateral scapula slide test are highly reliable in measuring the scapular position in swimmer. Purpose. The purpose of this study was to assess alteration scapular position in swimmers following high intensity swim training. Methods. 43 swimmers were participated in study. A repeated measures design used and two position of the LSST (lateral scapula slide test) were used. All measurements in each position were taken bilaterally. The LSST was administrated in participated swimmers at different level of training. All subjects had no present or past history of with a recent shoulder, cervical or thoracic injury or shoulder pain that interfered with swim training. Results. The data revealed statically highly significant difference for LSST position 2 on right side (p=0.001) and left side (p=0.002).and highly significant for LSST position 3 on left side (p=0.001) and significant on the right side (p= 0.028). Conclusion. The present finding suggests that high intensity training programme was effective an alteration in scapular position at different level of swim training in swimmers.
