Aim: To investigate the relation between the metabolic syndrome (MetS), and oxidized low density lipoprotein (ox-LDL) and non-high density lipoprotein cholesterol (non-HDL-C), and to evaluate a dietary therapy composed of a balanced hypocaloric regimen plus a supplement prepared from whole wheat and tigernut flour (Cyperus Esculentus). Material and Methods: Forty two obese Egyptian female volunteers (their mean age was 47.76 ± 0.90) were given a low caloric balanced diet (900-1000 kcal) for 4 weeks, and then the supplement was used to replace Baladi bread during the next four weeks. The metabolic syndrome was diagnosed according to the International Diabetes Federation criteria. Results: The prevalence rate of MetS was found to be 76.2%.The metabolic syndrome was significantly associated with higher levels of total cholesterol (T.cholesterol), LDL-C, ox-LDL and non HDL-C. Significant and sustained reduction in the levels of fasting blood glucose and HbA1c, normalization of lipid profile, and decrease in the levels of ox-LDL were observed after the end of the two periods of the dietary therapy. Conclusion: Metabolic syndrome is associated with higher levels of circulating ox-LDL and non-HDLC. The use of the dietary therapy composed of balanced hypocaloric regimen plus a supplement rich in antioxidants and anti-inflammatory components, reduced obesity and the risky biochemical markers of the MetS.

Aim: The present study was aimed to synthesize a series of 2-substituted benzylidine imino-3-(3-chloro-4-fluorophenyl)-carboxamido-4,5-trimethylene thiophenes SPJ-1(a-m) and to evaluate their in-vitro anti-inflammatory activity. Materials and Methods: The starting material (SPJ-1) was prepared by the application of versatile Gewald reaction. The In-vitro anti-inflammatory activity of synthesized compounds [SPJ-1(a-m)] was evaluated using inhibition of bovine serum albumin denaturation method. Result: SPJ-1b and SPJ-1g have shown significant in-vitro anti-inflammatory activity. Conclusion: The findings of the present study clearly demonstrate that chloro functional group possess inhibition of bovine serum albumin denaturation capacity and has in-vitro anti-inflammatory activity. However hydroxyl, nitro, methyl, methoxy and dimethyl amino derivatives did not show any in-vitro anti-inflammatory activity.

Background: Anatomical lung resection offers the best chance of cure for patients with localized lung cancer. Very often late diagnoses, advanced stage of the disease limit radical anatomical surgical resection. Use of neoadjuvant chemotherapy made some of the cases operable, and later they were surgically treated. Aim: to evaluate early (surgical) complications at patients with neoadjuvant therapy. Material and Methods: We compare 36 patients surgically treated with lung resection, before treated with neoadjuvant chemotherapy (3 to 6 cycles with platinum based regiments under protocol) with 42 patients surgically treated without neoadjuvant chemotherapy, in the same period, in the 2009 and 2010. Results: At the group with neoadjuvant chemotherapy we register more intraoperative and early postoperative complications as: bleeding 10 vs. 7, p=0.63, changes in lung vessels 3 vs. 0, p=0.8, prolonged air leak 9 vs. 6, p=0.63, stump fistula (at pneumectomy patients) 2 vs. 0, p=0.76, pneumonia 9 vs. 3, p=0.86, wound infection 2 vs 0, p=0.73, atelectasis 8 vs. 2, p=0.88, prolonged pleural drainage 10 vs. 4, p=0.87. There, also, was no difference in 30 days mortality rate between groups. Conclusions: Neoadjuvant therapy increased the perioperative complications in this group of patients compared with a similar group undergoing anatomical lung resection in the same institution. The most common complication in patients receiving induction chemotherapy was detected at the group with neoadjuvant chemotherapy. Strategies to prevent these complications will be important, especially if chemotherapy before resection becomes the standard for all patients with non–small cell lung cancer.

A surgery of ritual circumcision in healthy young boys is usually a safe procedure. However, an outbreak of severe sepsis a few hours after surgery in patients who underwent this minor procedure is described and analyzed in this report. We describe the clinical course and discuss the causes of the sepsis and septic shock. Contamination of propofol, the intravenous anesthetic agent, was suspected as a probable cause. However, the most important question that had appeared from this case is the prognostic outcome regarding the delayed treatment of severe sepsis.

Aim: A survey to address psychiatric morbidity in older adults admitted with hip fracture, involvement of liaison psychiatric services for older adults and evaluate change of level of care. Methods: Information about mental and physical health prior to and during admission, referrals to Geriatrician or Liaison Old Age Psychiatry (LOAP) team, duration of hospital stay and level of care was collected for 43 consecutive admissions of older adults for hip fracture over 6 months period. Results: Depression, dementia and delirium were present in 28%, 42% and 23% patients respectively. A pre admission diagnosis of depression was associated with lower Barthel scores on admission, whereas dementia and delirium with lower Barthel scores on both admission and at 48 hours post surgery. Pre admission diagnosis of depression or dementia and the occurrence of delirium did not affect the duration of stay in the orthopaedic ward or alter the level of care after discharge. Conclusions: There are high rates of psychiatric morbidity in older adults presenting with hip fracture to orthopaedic services. Active involvement of the LOAP team would facilitate their prompt identification and treatment post fracture and improve outcomes for these patients.

Many health-promoting and curative properties attributed to it are the basis for some traditional folk medicine treatments throughout the world today. Its beneficial effects in different disorders, rediscovered in recent decades , varying from its antibacterial effects and benefits in wound healing to its safe role in peptic ulcer, gastroenteritis, oncology, ophthalmology, dermatology and dental hygiene. This will be discussed in this review on the basis of a series of scientific studies conducted to investigate the therapeutic properties of this natural product.
