Socio-demographic Characteristics of Patients with Tuberculosis Spine in Bangladesh

Background: Spinal tuberculosis is more frequently reported in developing countries. Objective: The purpose of the present study was to see the socio-demographic characteristics of spinal tuberculosis patients. Methodology: This study was carried out in the Department of Neurosurgery, Dhaka Medical College Hospital during the period of January 2002 to December 2004 to elucidate the effectiveness of surgical and conservative treatment of the tuberculosis of the spine. For this purpose, a total number of 50 consecutive cases admitted during the study period with tuberculosis of the spine supported by investigations were included in this study. Result: The mean age of the patients was 25.4 years and ranged from 7 years to 68 years. The highest incidence of tuberculosis of the spine was found in age group between 11 to 20 years and 70% of the patients were under 30 years of age. Incidence of tuberculosis of this spine was more in male (62%) in comparison to female (3 8%) with a male female ratio of 1.63:1. In this study most of the patients came from low class society (66%) followed by middle class (28%) and high class (6%) respectively. Conclusion: Spinal TB more frequently occurs among the young age group with a male predominance.

Emergence of Vancomycin Resistant Staphylococcus aureus during Hospital Admission at a Tertiary Care Hospital in Bangladesh

Background: Glycopeptides such as vancomycin are frequently the choice of antibiotics for the treatment of infections caused by methicillin resistant Staphylococcus aureus (MRSA). For the last 7 years incidence of vancomycin intermediate S. aureus and vancomycin resistant S. aureus (VISA and VRSA respectively) has been increasing in various parts of the world. Objective: The present study was carried out to find out the presence of VISA and VRSA among isolated MRSA strains. Methodology: This cross sectional study was carried out in the Department of Microbiology in Dhaka medical college during period of January 2010 to December 2011. All S. aureus isolates were screened to detect methicillin resistance and then all MRSA isolates were subjected for MIC testing against vancomycin and oxacillin by agar dilution method, disc diffusion testing and PCR for mecA and pvl genes detection. Result: A total 112 S. aureus were isolated from 500 nasal swab sample collected from adult patients who were admitted in various departments and wards in Dhaka Medical College Hospital. Among 38 MRSA strains out of 112 Staph aureus isolates 3(7.89%) strains were resistance to vancomycin of which 2(5.26%) strains had MIC > 256 mg/mL and one strain had MIC 256mg/mL. All vancomycin resistance strains had MIC of oxacillin > 256 mg/mL. All isolates possess mec-A gene. Conclusion: The present study reveals that emergence of VRSA upon admission at a tertiary care of hospital in Bangladesh. Continuous efforts should be made to prevent the spread and the emergence of VRSA by early detection of the resistant strains and using the proper infection control measures in the hospital setting.

The relation between ZIKV infection during pregnancy and microcephaly in neonate has been established by the Brazillian Ministry of Health. Therefore, more attention regarding control of Aedes mosquito is required. Since it transmits this disease, which has more disastrous consequences than dengue viral infection. Potentially effective methods of prevention that are focused on reducing infections among pregnant women include avoiding unnecessary travel to areas of ongoing Zika virus transmission, avoiding unprotected sexual contact with partners who are at risk for Zika virus infection. According to Institute of Epidemiology Disease Control and Reseaech (IEDCR) there is nothing to panic at this moment for Bangladesh, but they stressed on mosquito control, which is the only tool to prevent and fight off Zika virus. They however, could not mention how the virus came to Bangladesh. May be somebody brought it while travelling. There is good survillence and testing system to detect the disease in Bangladesh.
