Efficacia del trattamento manipolativo osteopatico sulla forza muscolare degli arti superiori e inferiori: studio clinico controllato, randomizzato crossover

Introduzione. Il ruolo della forza muscolare è riconosciuto per ottimizzare la salute, la funzione, l'indipendenza, e sono legati al suo miglioramento e mantenimento nel tempo. Gli obiettivi di questo studio sono stati esplorare l'influenza del trattamento manipolativo osteopatico (TMO) nel breve termine sulla forza muscolare in soggetti sani, rispetto alla terapia simulata (Sham) del gruppo di controllo Metodi. 76 soggetti, maschi e femmine, sono stati inclusi nel presente trial clinico placebo controllato entro i soggetti, randomizzato in 2 bracci crossover in triplo cieco. I partecipanti erano adulti asintomatici (35 ±10 anni, il 61% di sesso maschile, BMI 25 ±2,55). I soggetti sono stati randomizzati in due gruppi: sperimentale a ricevere la sequenza intervento TMO-Sham e controllo a ricevere la sequenza Sham-TMO. La valutazione osteopatica, la somministrazione di TMO e Sham è stata praticata nei gruppi durante le sequenze assegnate, seguito da un protocollo di TMO aggiuntivo standardizzato Principali misure di esito. I cambiamenti della forza sono stati misurati con il test dei piegamenti delle braccia Push Up (TF1), e il test del salto in alto da fermo di Abalakov (TF2) rilevando i punteggi medi pre-post intervento durante un periodo di 7 settimane, a 3 settimane e 7 settimane, la 4 settimana ha compreso la fase di interruzione del trattamento (washout) Principali risultati. Le analisi tra gruppo sperimentale e controllo a 7 settimane entro le sequenze intervento TMO-Sham / Sham-TMO registrano nel 65 % del sottogruppo donne (TF1PRE-POST ) test t a campioni indipendenti variazioni significative di aumento forza (p<0.02) cohen’s d / effect size r (d. 1.73, r 0.65; IC 95% 1.16834 a 2.83166) vs intervento TMO Discussione. Il TMO produce cambiamenti della forza nelle donne della muscolatura di arti superiori come bicipite, tricipite, pettorali come dimostrato da un aumento di 1-2 piegamenti sulle braccia rispetto alla terapia simulata del gruppo controllo Conclusione. I risultati suggeriscono che il TMO influenza positivamente la forza degli arti superiori nel breve termine in donne sane. Futuri studi sono giustificati per investigare la misura in cui il TMO possa cambiare la forza anche in condizioni patologiche. Study Design. Studio clinico controllato, randomizzato crossover. Association of body mass index with flexibility in adults

Objectives: To determine the frequency of obesity in normal adults and find association between flexibility and body mass index. Study Design: A cross-sectional study was done using non-probability convenient sampling. Place & Duration of Study: The study was started in February 2019 –July 2019. The data was collected from universities and hostels of Islamabad Capital Territory. Material & Methods: A total of n=295 female and n=53 male participants who fulfilled the selection criteria were included in the study. A self-structured questionnaire was used. Data was analyzed using SPSS software. Results: 348 individuals participated in the study; most of them were university students. Females (84.8%) were in greater number as compared with males (15.2%) and mean age of the participants was 21 ± 2.4 years. The results showed an association of body mass index (BMI) with flexibility. Knee ranges were affected mostly by increase in body mass index because muscle bulk was greater and resulted in restricted range of motion. Most of the individuals had normal flexibility irrespective of body mass index. Hip flexion was decreased mainly due to less use of hamstrings. Upper limb had greater flexibility because it is used more in proportion to lower limb. Conclusion: The study concluded that there is significant association between flexibility and body mass index. The study found that 3.7% adults were obese and 0.3% were morbid obese. Study Design. A cross-sectional study was done using non-probability convenient sampling. In an Nursing home the number of individuals with an severe stroke with the Pusher syndrome are much more than in Rehabilitation center. This person with an severe Pusher syndrome are present on the geriatric rehabilitation ward but also on the “Longstay” ward. In the hospital we know that almost all person with stroke has moment of an pushing behavior, almost 90%, but most signs are gone within an day. By the people where the signs are manifest an little group goes to the Rehabilitation center (10%) and the other person will be trained in long care facilities. This article want to give an view, what the possibilities are for this stroke patient to get the best treatment, well pointed on the level of disability. The person will have an difficult start in the hospital and will have much more time needed to restore for he can make an attempt to recover. But investigation are clear that when they regain their condition that the spontaneous recovery start as well as by all other stroke survivors. Still there are many reason why an patient cannot recover and that is something different as not capable to learn or re-learn or take part in an treatment. Most often the condition is so poor that there is no energy for more than only survive. On the moment that the condition is better, every patient can learn but now it is dependent of the therapy that must be on the right level and the good motoric learning method. That ask for an good understanding, what the “pusher syndrome “is and what is important to change before the person can take part in an common rehabilitation program. Famous physical therapist out the N.D.T. –Bobath concept have developed an approach that has convince many physical therapist and is done all over the world. Her work is that she discovered the person with the Pusher syndrome and give its place but also created an treatment for this persons with an severe stroke. And even with severe Pusher it works, when this person has the physical capacity to exercise. In this article the person with an severe stroke stand central, but the person with moderate signs will be easier to do. So it is obvious that working with severe disable people makes working with moderate affected people will have less “problems” to do the exercise and get this people on higher level. Background. L'Osteoartrite (OA) determina alterazioni patologiche irreversibili alle articolazioni inducendo a sintomi invalidanti come dolore, rigidità, instabilità, deformità, gonfiore, crepitio, con limitazioni di attività quotidiane e sportive. I fattori di rischio, per lo sviluppo di OA, sono ben noti e includono età, sesso, obesità, lesioni articolari e carichi anomali sulle articolazioni. Obiettivo. L’obiettivo primario di questo studio è stato quello di sintetizzare la letteratura presente per esplorare la complessità, prevalenza e incidenza di OA negli ex atleti d'élite nel calcio attraverso una descrizione epidemiologica. L’obiettivo secondario è stato quello di indagare per la condizione clinica di OA i criteri scientifici della somministrazione di acido ialuronico, supplementazione con vitamina D e integrazione nutrizionale. Metodi. Una procedura di ricerca bibliografica senza limiti di data e filtro metodologico, con la combinazione di Medical Subject Heading e testo libero è avvenuta online nel periodo compreso tra Gennaio - Maggio 2019 nel database elettronico PubMed. Risultati. Dei pertinenti abstracts sono stati recuperati (n° 90) articoli, dei quali (n° 62) ritenuti idonei per questa revisione. Gli articoli ascrivibili nella piramide delle evidenze sono Meta-analisys (n° 2), Systematic reviews (n° 11), Randomized controlled trial (n° 6), Observational study (n° 1). I restanti studi erano con disegno non dichiarato, o non chiaro definiti come Review (n° 25), Review guideline (n° 2), Research support (n° 14), Pilot Study (N°1). Discussione. I principali risultati hanno evidenziato come i calciatori professionisti in attività e in pensione avevano quasi il doppio delle probabilità di soffrire di OA del ginocchio e all’anca sia per eventi lesivi traumatici, sia per l’alto impatto della gestualità tecnica - atletica. La terapia infiltrativa con acido ialuronico, è suggerita come l’attuale terapia di elezione nell’atleta con OA. Essa riscontra un ampio consenso nella letteratura nel decrementare il dolore e la funzione articolare, migliorando così la performance in atleti, ancora in attività, con OA. Relativamente alla vitamina D il 63% dei pazienti con OA del ginocchio ha mostrato un basso livello ematologico della vitamina D, e la sua integrazione, come supporta questa narrazione, riduce il dolore e migliorare la funzione articolare, anche se non ci sono prove ben definite sugli effetti, della stessa, sulla cartilagine articolare. Pochi studi hanno dimostrato un effetto protettore della dieta mediterranea, nell'OA, con dati di prova ancora limitati. Conclusioni. I calciatori hanno un rischio maggiore di sviluppare OA, in particolare post traumatica del ginocchio e dell’anca. Un’identificazione precoce, nell’atleta in attività di un OA del ginocchio e/o dell’anca, dovrebbe, attraverso l’utilizzo di un management clinico / riabilitativo con un’idonea supplementazione, indirizzare a una migliore gestione del quadro sintomatologico e funzionale. Ulteriori studi, con rigore metodologico, devono essere garantiti in futuro per dare risposte basate su dati di prova. Study Design. A Narrative Review. Authorship Credit: “Equal Contribution” (EC).
