Background.The important biomechanical role played by the meniscus in the functionality and stability of the knee during sports performance is known in the literature. The combinations of torsional and axial load appear to be the cause of most meniscal injuries. Diagnosis of acute knee injuries has long been a topic of discussion throughout orthopedic literature. Many clinical tests and diagnostic studies have been developed to increase the clinician's ability to accurately diagnose these types of knee disorders. Objective. The objective of our study is to verify through a review of the literature if Clinical tests in a closed kinetic chain to diagnose the meniscal knee lesions have a validity, if they are correlated with the classical tests in the open kinetic chain and then propose them as a screening tool in the their practical applicability on the pitch during a competition or training. Materials and methods. In this systematic review of the literature we used the Search Equation of the Boolean Operators AND and OR within Search Equation. We have thus inserted specific Key words to the research topic: physical examination, meniscal tears, Physical Examination for Meniscus Tears, Clinical diagnosis of meniscal tears, tests for meniscal Injury specific to the research topic. At the word "meniscus tear" we found, in December 2018, on PubMed 3814 abstracts and full-text articles. While on PubMed Central 4737 from which we have considered, it is our search only 841 for free Full-text articles. To the keywords "Meniscal test" highlighted 1068 abstracts and Full-text articles on PubMed and 4986 abstracts and Full-text articles PubMed Central. We have tried to further refine our research by addressing it in the last 5 years with 417 articles published on PubMed. Of these we have selected, for our research, 166 free full text articles. Results. For the preparation of this review have selected 57 scientific articles. Recently, from the international scientific movement, tests have been reported, in a closed kinetic chain, that reproduce technical-athletic gestures, for the diagnosis of meniscal lesion. Among these the Thessaly Test and the Ege's Test, have shown a high diagnostic accuracy, approaching, in fact, reference models such as joint line tenderness, McMurray's test and Apley's compression test. Among other things, these tests, in CKC, stimulating technical / athletic gestural moments, these tests can find, "going towards the lesion" an indication and prescription both in rehabilitation training and in physical ones aimed at prevention, given the extremely articular environment proprioceptive, knee ligament capsule injuries and, in particular, meniscal lesions. We did not find any work related to the practical applicability of these clinical tests used during competitions and training. Conclusions. Meniscal tears are frequently found in sports. The diagnosis can be accurately performed in 75% of the knees on the basis of the clinical history alone, while the specific clinical tests that have been used for the detection of these lesions do not have high values of sensitivity and specificity if taken individually. In our study we have identified test clinic in closed kinetic chain that allow to operate a more incisive evaluation, correlating clinical expressivity with the specific gestures of the athlete. The change that this will succeed is very limited. In the Netherlands the approach EDOMAH is investigated and that show that it is possible when there is an conversation between the individual and the family about their wishes. And the wishes are the start for an better treatment plane and that is often, at the start, not the conclusion/wishes of the professional. This process, that people talk with each other and – maybe the most important issue- have confidence in the expertise of the professional, lead often to an situation that exercising and adaptation of the environment to get an better independency of the individual with dementia. In the Netherlands are family doctors starting with an approach that able the individual and his/her family to stay at home and get the family doctor at home on regular base to discuss the problems and what has to be done. In this approach are other professionals introduced by this family doctor and one of the most important elements, is that the people that comes at home are always or almost always the same and there is base that is needed to get an optimal quality of live for this people. Often is an assessment impossible the first time but with an introduction of the family doctor that was now often easy and could also discuss with the other professionals and the individual with dementia and his/her family. There we have the base and now we can start to change /adapt the environment (family and bed/toilet etc.) and introduced the treatment plane that can be done on an regular base. Background– Le lesioni del Legamento Crociato Anteriore (LCA), sia all'innesto ipsilaterale utilizzato per la ricostruzione del legamento crociato anteriore (LCA), sia una nuova lesione all'ACL controlaterale, sono esiti nefasti, per lo status psico-fisico dell’l’atleta che ritorna all'attività sportiva. Gli studi che riportano le percentuali di re-injury del LCA, nelle popolazioni attive, sono presenti in letteratura, ma questi dati non trovano, oggi, sinergie scientifiche. Sfortunatamente, la chirurgia ricostruttiva, finalizzata alla stabilità del ginocchio, non garantisce che tutti i pazienti tornino al loro livello di performance pre –injury. Le ragioni per cui alcuni atleti non tornano ai precedenti livelli di attività, sono diversificate e le nostre conoscenze scientifiche sono limitate. Recentemente, è stata sottolineata l'importanza dei fattori psicoattitudinali alla pratica sportiva. Uno di questi fattori, la Kinesiofobia o la paura di incorrere in una re-injury, può giocare un ruolo significativo, nell'incapacità di alcuni pazienti di tornare con successo allo sport ed al livello di performance pre–injury. Scopo. L’obiettivo di questo studio è stato quello di utilizzare, a completamento del follow up a 6 mesi, dalla chirurgia ricostruttiva del LCA, la Tampa Score of Kinesiophobia (TSK 17), per verificare, il livello di adattabilità psicologica sul ritorno allo sport. Materiali e Metodi. Dal gennaio 2014, al giugno 2018, sono stati valutati, con un follow – up a 6 mesi dalla chirurgia ricostruttiva del LCA, 181 pazienti. Furono abilitati per lo studio, rispettando i criteri di inclusione,86 uomini (età media 28±06 anni e altezza media di 178±05 cm) e 14 donne (età media 24±02 anni, altezza media di 172±03 cm). I 38 pazienti, risultati positivi al Single Leg Squat test, furono invitati, dopo il test, a compilare sia il questionario Tampa Score of Kinesiophobia nella sua versione inglese a 17 items, per valutare il profilo psicologico di adattabilità alla gestualità semplice e complessa, sia la VAS score, per monitorare il livello del dolore durante il test dinamico. Risultati. Dei 100 pazienti operati di LCA, 38/100 presentavano risultavano positivi al Single Leg Squat Test (SLST). I 38 atleti (38%) presentavano un decremento associato, del controllo posturale del cingolo pelvico (pelvic drop controlaterale sul piano frontale) e del ginocchio (valgo dinamico adattativo, osservabile sul piano frontale, durante l’esecuzione del test). L’elaborazione del questionario Tampa Score of Kinesiophobia (TSK.17), ha evidenziato condizioni psicologiche, con alte percentuali di Kinesiophobia ed una media totale di punteggio, pari a 50.1± 4.7 con 30/38 (78,9%) pazienti che all’item 1 della TSK evidenziavano un altissimo livello di kinesiophobia. La Visual Analogue Scale (VAS) for pain presentava una media del Pain Level di 1.5 ±1.9, con 19/38 pazienti che riferivano un’assenza della sintomatologia dolorosa. Alla luce dei risultati della Tampa Score e della VAS non è possibile correlare il dolore, visti i bassi livelli, con un incremento di Kinesiophobia. Sicuramente un follow up, identico, a 12 e 24 mesi ci permetterebbe di indentificare eventuali re-injury ipsilaterale o controlaterali, durante il ritorno allo sport e, quindi, arrivare a risultatati che possono avere una validità di indirizzo scientifico. Possiamo affermare, però, che, sicuramente, l’alto livello di kinesiophobia, può condizionare l’espressività gestuale tecnico-atleta semplice e complessa dell’atleta. Conclusioni. La Tampa Score of Kinesiophobia a 17 items, associata alla VAS, può essere un appropriato strumento, per valutare il profilo di adattabilità psicologica durante il ritorno allo sport post ricostruzione del LCA. Questo nostro studio sottolinea, come esiste, nei 38 pazienti esaminati, una correlazione associativa, tra decremento del controllo posturale del cingolo pelvico e valgo adattativo dinamico del ginocchio, a 6 mesi della ricostruzione del legamento crociato anteriore ed un alto livello di Kinesiophobia. In contrapposizione, la mancanza di omogeneità del processo riabilitativo, non ci ha permesso di effettuare uno screening, sull’incidenza della riabilitazione, sui risultati della Tampa Scale of Kinesiophobia e VAS. Dobbiamo comunque sottolineare, attraverso un’analisi soggettiva deduttiva, nella nostra sintesi conclusiva, come alti livelli di Kinesiophobia, evidenziati, negli sportivi analizzati in questo studio, possono rappresentare un chiaro fattore di condizionamento dell’espressività gestuale, con un incremento dei fattori di rischio di re – injury. Aim of research was to examine the effects of consuming CR to muscle strength and body composition. Duration of experimental treatment was 8 weeks. Sample consisted of 14 participants aged 24±6months divided into two sub-samples: 6 participants who consumed CR and 8 participants who were placebo group. Strength was assessed using 3 tests: Bench Press 1RM, Leg Press 1RM, and Barbell Biceps Scott – maximum number of repetitions with 15kg load. Following parameters were obtained for the purpose of assessment of body composition: Body mass, Muscle mass, Body Fat, Body Water. Assessment of body composition was performed using bioelectric impedance TANITA BC-545n. Differences between the groups were assessed by ANOVA test of repeated measures. Upon taking insight into the results we may conclude that CR influences changes in body composition: Body mass (.000), Muscle mass (.039), Body Water (.010); effects to Body Fat are not statistically significant. In placebo group changes were noticed only in variable Body Water (.007). Effects to body strength were confirmed in all three variables: Bench Press (.050), Leg Press (.041), Barbell Biceps Scott (.003)., whereas no significant changes were observed in placebo group. CR is efficient dietary supplement for both professional and amateur athletes. Citation. Objective. To give an example what can be an good treatment to counter the Pisa-syndrome by persons with the disease of Parkinson. Method. To give an overview of the assessment and possible treatment possibilities that are the outcome of that assessment. And an search in the literature to find an way that the shortening of the muscle of the spine is treatable. And place that possibilities in the treatment session but also an attempt to bring this change in the ADL of this person. So that the complaints of him has an possibility to be counter also during the day. Results. The greatest result were there, when he was exercising in the water but there was only an short effect of this on land. Through the hypothesis of the effect in water and the discovery of the dynamic proprioceptive /exteroceptive stimuli was it possible to get an grip on the shortening of the spine on land and not only in the treatment but also on some moments in de ADL. Especially the speech/ eating/swallowing component was improved. Conclusion. One case can never be an significant prove that this approach is working but it has learn us that the shortening of the spine ( Pisa –Syndrome) has his roots in an “malfunction” in the brain. That brain is treatable through the right stimuli and will react with an decrease of tone /rigidity but only for an short time and with the possibility to give dynamic stimuli. Background. Very often the anthropometric parameters (height and mass) are primarily used in the assessment of the morphological status of an individual. Morphological, space is often the subject of a research in the field of physical education and sports. Their analysis can detect certain psychosomatic changes, the trend of their development and possible differences. Purpose.The aim of the research was to analyze the somatic status of students of middle primary school age. Methods. The sample has included a population of 68 primary school students from primary school ″Serbia″ in East Sarajevo (Bosnia and Herzegovina), aged 11±0.5. Two variables body height (AVIS) and body mass (AMAS) have been used to analyze the morphological area. Results. The results of the study have shown that boys and girls are identical in height and weight. Conclusion. This is an contracture !! We have heard this “Often and almost every time too fast”, That “ An restriction in movement is called an contracture”. This give the physical therapist and others an tool to say: “ That this isn’t treatable”. But is every contractures an irreversible situation and therefore not treatable ? Working in an nursing home, there were so many situations in which there was an “contracture” and always was the reaction, that this wasn’t treatable. Of course, there were joint deformation, that makes an movement not possible and the only cure was and is an joint replacement. But in the nursing home, the most people suffer often from an neurological disease and one of the most remarkable symptoms is the increased tone, together with the loss of selectivity, gives attitudes that had an great “resemblance” with the joint destruction symptoms. That has lead, that this attitude with high tone was called an contracture and regrettable give that almost always the same reaction : “Irreversible and not treatable ”. People with an neurological disease with great loss of control (selectivity) can suffer from loss of mobility, but often it was possible to find, what this loss of mobility caused. Par example an restriction in the extension of knee was caused by the high tone in the hamstrings muscle. Try of change this with stretch exercise, that will give no or almost no reaction, but the reason was and is high tone, that don’t react on stretch alone. That means only, that this therapy isn’t correct or incomplete. And there must be an good assessment, WHY the tone of the hamstrings is so high and not react ? Observe an Individual with an neurological disease and an high tone in his hamstrings and this person has an epileptic insult. He lost his conscious and the tone decrease fast and the restriction ( “the contracture” ) is gone. After that, when he is coming back, the tone will come back. Background: L’utilizzo della time motion analysis attraverso l’utilizzo dei GPS e della video match analysis ha permesso, nel corso del tempo, di delineare sempre più nel dettaglio il modello prestativo del calcio, delineando così i parametri di carico esterno durante le partite. Obiettivo: L’obiettivo di questo studio è stato quello di rappresentare un modello di prestazione di una squadra giovanile di calcio mediante l’utilizzo di dispositivi GPS a 10 HZ, la cui validità e affidabilità di rilevazione è stata dimostrata. Materiali e metodi: Alla sperimentazione hanno preso parte quindici calciatori adolescenti, divisi in tre gruppi così organizzati: difensori (D, n = 4), centrocampisti (C, n = 7) e attaccanti (A, n = 4). I soggetti facevano parte della stessa squadra, partecipanti al campionato Nazionale della categoria Giovanissimi, nell’annata calcistica 2016/2017. Ciascun giocatore ha indossato un dispositivo GPS consentendo l’analisi dei dati rilevati mediante il software LagalaColli. Risultati: La distanza totale, percorsa in media dalla squadra, è stata di 8138 ± 367,9 m (4074,5 ± 330,8 m nel primo tempo; 4063,9 ± 404,9 m nel secondo tempo), con una spesa energetica media di 47,9 Kj/Kg (24,0 ± 1,9 Kj/Kg nel primo tempo; 23,9 ± 2,6 Kj/Kg). I C sono coloro che hanno percorso una maggiore distanza totale media (8612,4 ± 252,4 m) rispetto a D (7571,0 ± 348,6 m; p=0,0003) e A (7571,0 ± 348,6 m; p=0,0257). La potenza metabolica media durante la partita è stata di 10,8 ± 1,1 W/Kg (11,2 ± 1.0 W/Kg nel primo tempo; 10,3 ± 1,1 W/Kg nel secondo tempo). Il confronto tra ruoli relativo alla potenza metabolica ha mostrato l’esistenza di una differenza statisticamente significativa nel confronto tra D e C (rispettivamente 9,9 ± 1,0 W/Kg e 11,5 ± 0,7 W/Kg; p=0,0005) e in maniera minore anche tra C e A (rispettivamente 11,5 ± 0,7 W/Kg e 10,5 ± 0,9 W/Kg; p=0,0362). Non sono state riscontrate differenze statisticamente significative tra D e A (p=0,2758). Conclusioni: I risultati di questo studio hanno permesso di classificare la performance fisica di una squadra giovanile di calcio per cercare di creare un nuovo modello di prestazione analizzando il dispendio energetico, l’impegno neuromuscolare e applicando il modello della potenza metabolica non solo alle categorie di potenza ma anche alle varie fasi di gioco. Inoltre, si sono valutate le differenze prestative a seconda del ruolo tattico, oltre che le differenze di prestazione tra primo e secondo tempo. Obiettivo: L’obiettivo dello studio è stato quello di correlare i risultati ricavati con il test di Gacon (45”/15”) rispetto ai valori cinematici rilevati durante le gare della pre-season in un gruppo di calciatori professionisti di una squadra U-19 di Serie A (Primavera 1). Materiali e metodi: Lo studio ha coinvolto 13 calciatori di differenti ruoli tattici con due requisiti necessari; aver svolto il test di Gacon e almeno 43 minuti di almeno un match della pre-season. Il test di Gacon è stato eseguito durante la prima fase della preparazione, nel mese di luglio 2017.Durante tutte le gare, prese in esame, i calciatori sono stati dotati di un dispositivo GPS con frequenza di campionamento a 10Hz (Qstarz modello BT-Q1000Ex, Taiwan). Le rilevazioni in gara hanno previsto l’analisi di un totale di 77 prestazioni individuali rilevate durante le amichevoli pre-stagionali. Al temine di ogni gara i dati ricavati sono stati scaricati e analizzati attraverso il software LagalaColli 10.03 (Spinitalia SRL, Rome, Italy). Risultati: La media di squadra della VAM, emersa dopo lo svolgimento del test di Gacon, è di 18,7 ± 0,78 km/h. Durante le gare prese in esame sono stati analizzati differenti parametri cinematici; come quello in riferimento ai watt di potenza metabolica quantificati in 10,9 ± 0,98w. Le distanze al minuto e le distanze al minuto equivalente sono state rispettivamente 112,3 ± 9,33 metri e 138,2 ± 12,41 m. La percentuale di lavoro svolto sopra la VAM durante le partite analizzate è stata del 5 ± 1%. La percentuale media di energia totale prodotta per via anaerobica, calcolata tramite il prodotto di quantità di tempo per potenza espressa maggiore di 20w è stata di 37 ± 3%. La percentuale di lavoro svolto sopra la VAM per la durata del tempo totale è stata del 14 ± 2%. Le distanze al minuto e le distanze al minuto equivalente sono state rispettivamente 112,3 ± 9,33 metri e 138,2 ± 12,41 m. Il numero medio di azioni intense in gara, che difficilmente superano i 3 secondi, sono state di 3,6 ± 0,57. Il numero di tempi di recupero passivi al minuto registrati sono stati 16,30 ± 3,6. Il numero di tempi medi di recupero passivo in un minuto compresi tra 0-20, 21-40, 41-60 e oltre i 60 secondi sono stati rispettivamente 6,7 ± 2,48; 4,6 ± 1,12; 3,2 ± 1,43; 3,2 ± 1,04. Conclusioni: La VAM emersa dal test non può essere valutata come indice di prestazione, come affermato da precedenti studi; il test di Gacon, infatti, eseguito durante le partite della pre-season, non mostra nessuna correlazione con i parametri di movimento rilevati durante la gara, ma è un test utile per valutare la condizione generale dei giocatori.
