The end of life concept need to be thought by the family members on the day the family member is diagnosed with dementia. The progress of dementia is unpredictable, hence one need not wait for the dementia to progress and wait until last stage sets in. The concept of end of life care that it should begin during terminal stage will not hold true in person with dementia as it is true in people living with cancer. The care givers being Nurses should insist the person with dementia to share their wishes and any specific needs to be met. The end of life care begins from the day of diagnosis of dementia and continues even after death of person, involving care givers. This article discuss on the issues related to person with dementia and the care giver who happen to be a Nurse. This relation need to be coordinated, emphasized, the issues need to be addressed and provide comfort to both.

Functional appliances are passive appliances, which make use of the natural forces generated by the orofacial & masticatory muscles and the forces of occlusion to bring about changes in dento alveolar & craniofacial structures. These appliances alter neuromuscular environment of orofacial region to improve occlusal development and craniofacial skeletal growth. In this article we will discuss effects of two functional appliances that are activator and bionator.

Aim: Assessment of left ventricular diastolic function by transmitral doppler and Left atrial volume estimation. Materials and Method: This perspective cross sectional study was conducted at AVBRH, JNMC, Sawangi, Wardha, which is an 850 Bedded hospital with facilities for tertiary care. The study was carried out in 200 patients (100 cases and 100 controls) Study Group comprised of patients with risk factors like diabetes mellitus, hypertension and ischemic heart disease. Control group were devoid of any risk factors. Results: In our study we found that mean e/a ratio in the study group was 0.93 ± 0.13 while that in the control group is 1.14 ± 0.11. The difference was statistically significant. (p = 0.0001), Mean e wave deceleration time (DT) in the study group was 150.26 ± 13.88 while that in the control group is 138.71± 13.79 and the difference between these two groups was statistically significant. (p = 0.000), and Mean isovolumetric relaxation time (IVRT) in the study group was 95.44 ± 9.74 while that in the control group is 86.50 ± 8.90 and the difference between these two groups was statistically significant. ( p = 0.000), Mean left atrial (LA) volume in the study group was 29.38 ± 2.34 while that in the control group was 26.01 ± 1.41 and the difference between these two groups was statistically significant. Conclusion: The study of transmitral Doppler and 2 D echo variables revealed that, e/a ratio, e wave DT, IVRT, LA volume demonstrated significant change towards abnormal in subjects with presence of risk factors i.e. HT, DM, IHD either single or multiple. As regards the two modalities (Transmitral doppler and 2 D Echo) it was observed that both are equally effective in detecting LV diastolic function abnormalities.

Brooke-Spiegler syndrome (BSS) was reported for the first time in 1842 by Ancell.(1) It is an autosomal dominantly inherited syndrome, characterised by multiple skin appendageal tumors especially trichoepitheliomas and cylindromas and occasionally spiradenomas which usually appears in the second or third decade of life.(2) With just 60 cases of Brooke-Spiegler syndrome reported in the literature so far across the globe, the entity is considered very rare. Herein, we are reporting a case of Brooke-Spiegler Syndrome in 35 years old female.

Scrofuloderma is a common form of cutaneous tuberculosis, which commonly involves parotid, submandibular, subclavicular regions and the lateral neck. They are often misdiagnosed as cold abscess. Diagnosis is made by clinical examination, mantoux test, acid fast bascilli (AFB) culture, fine needle aspiration cytology (FNAC) or biopsy or polymerase chain reaction (PCR). We report a case of 19 year old male presenting with painful swelling on neck. He was successfully managed with antitubercular (ATT) therapy for 6 months.
