Obiettivi Lo scopo di questo studio preliminare è stato studiare l’efficacia e le modalità di erogazione di Cura Osteopatica Centrata sulla Persona (COCsP) in un campione di popolazione con Low Back Pain (LBP) e NeckPain (NP) Metodi Un disegno di studio pilota descrittivo con revisione retrospettiva di cartelle cliniche (CC) in un campione al basale di (N=220) CC presso il setting di pratica clinica privata osteopatica è stato progettato per la verifica delle seguenti variabili di interesse. End-points (clinico): variazioni punteggio medio campionario delle scale di misura dedicate per LBP e NP pre/post somministrazione di COCsP utilizzando l’analisi della varianza (ANOVA). End-points (surrogati): numero medio OMT, tipologie di tecniche osteopatiche somministrate e verifica combinazioni modelli osteopatici Struttura Funzione (SF) Risultati Rispettando i prestabiliti criteri di inclusione ed esclusione è stato osservato un campione finale di N=128 CC di cui: NP N=40 (31%) età media 39 (ds±15,45) e LBP N=88 (69%) età media 45 (ds±14,25). End point clinico ANOVA NP scala Neck Pain and Disability Scale (NPDI-I) N=40: (media campionaria-pre 28 [IC 95%: 23.8424 - 32.1576] media campionaria-post 10 [IC 95%: 7.12167 - 12.8783]) variazione(-18 punti %; p=.00). ANOVA LBP scala Oswestry Disability Index (ODI-I) N=72 (media campionaria-pre 26 [IC 95%: 24.1201 - 27.8799] media campionaria-post 10 [IC: 7.41514 - 12.5849]) variazione (-16 punti %; p=.00). ANOVA sottogruppo LBP scala Roland Morris (RM) N=16: (media campionaria-pre 7 [IC: 5.4014 - 8.59858] media campionaria-post 1 [IC: 0.467139 - 1.53286]) variazione (-6 punti %; p=.00). End point surrogato minimo (N=2 OMT), massimo (N=5 OMT), tecniche somministrate indirette (N=105[82%]), combinazione modelli SF Biomeccanico/Neurologico (N=105[82%]) Discussione I dati per l’end point (clinico) per la verifica dell’ipotesi di nessun effetto della COCsP è stata rifiutata e si può affermare che la cura è efficace con un effetto statisticamente significativo. I dati per l’end point (surrogato) registrano in media (N=3) sessioni di OMT con tecniche indirette e una maggiore combinazione dei modelli SF Biomeccanico/Neurologico Conclusione I dati significativi ottenuti per l’end point clinico e la comprensione sulle modalità di COCsP giustificano la pianificazione di studi clinici controllati randomizzati

An exploratory investigation into identifying the answers to two specific questions related to this condition.  Why do many individuals develop ‘striker foot’ following severe stroke?  What is the best intervention to help control its development? The answer to the first question lies within the lack of stability in the paretic leg when the patient attempts to move in bed using the other leg. With the second question the answer is less obvious, although there are indications that greater stability helps to maintain the muscular tone of the calf leading to better overall control. Throughout this investigation the only changing factor with stability was the mattress which suggested that more would be required to prevent ‘striker foot’ developing. Therefore further investigations are needed to gain a better understanding and help to reduce the numbers of severe stroke patients who go on to develop this condition.

Lo scopo di questo studio è valutare lo stato dell’arte degli aspetti clinici e del management terapeutico della sindrome patello-femorale (PFPS) in atleti runners tramite una revisione critica della letteratura biomedica. Metodi: Gli autori hanno effettuato una revisione della letteratura utilizzando Clinical Queries scegliendo una “BROAD STRATEGY” e utilizzando anche MeSHTerms combinati ad operatori Booleani “AND” e “OR” e “textwords” indipendenti nella banca dati biomedica PubMed, unitamente ad una ricerca per autori e “textwords” indipendenti nella banca dati scientifica Google Scholar, includendo anche estratti da monografie e studi non indicizzati in lingua inlese ed italiana. Risultati: Sono stati inclusi e visionati 100 studi relativi alla revisione in questione, ed alla applicabilità, sicurezza ed efficacia della gestione proposta. Conclusioni: Da questa nostra analisi della letteratura emerge come La sindrome patello-femorale ( PFPS) nei runners ha un indirizzo terapeutico funzionale assolutamente conservativo. I fattori chiave del processo riabilitativo dovrebbero essere indirizzati verso miglioramenti della funzionalità articolare del ginocchio,, forza, flessibilità ,controllo neuromuscolare , con un attenta progressione del ritorno alla attività sportiva.

Perception of the body is one way in which the brains makes direct contact with the outside world. This is essential information which following a stroke can present as a problem for individuals due to the differences in ability. It is important to identify what the brain “feels” and then interprets from the information which results in our external bodily responses. We are already aware that our vision and other senses can alter on two levels. These differences can be either in the sense itself or in the assimilation of the damage brain. This information assimilation provides the brain with the tools to react using the damage muscle system, especially damage in the selectivity process. Following a stroke there is always a degree of damage sustained in the diagonals, therefore it would be incorrect to differentiate by using of affect and unaffected side for identification purposes. However for the purpose of this article we do so to prevent the possible confusion this could pose to the reader. Damage to the diagonal makes lying and moving in bed more difficult. Within part 3 we will consider bed attitude which gives the individual a starting position to perform movements because now the brain can search for solutions. In severe cases it is often the lack of stability which results in the individual being unable to move sufficiently and therefore affects recovery. In this article we explain how to supporting the individual to start developing this stability and work towards building-up movement whilst in bed.
